Diaper Assistance Program
According to the National Diaper Bank Network, 1 in 3 families struggle to provide clean diapers for their baby. // (National Diaper Bank Network | Meeting the Basic Needs of All Families in America., 2015a)
The CADC Diaper Assistance Program (DAP) is a supplemental program designed to assist low-income families by reducing the diaper need within Central Arkansas.
With this program, families are able to receive up to 6 packs of diapers (3 packs with each visit) throughout the duration of an open program to assist with their diaper needs.

The purpose of this form is to capture your basic information to provide you with diaper assistance! Please visit your local CADC office for an application. When applying, be sure to complete the form in its entirety, including providing a valid phone number and email address.

Eligibility Guidelines
Diaper Assistance Program Eligibility is based on 150% of the Federal Poverty Guidelines.
